Care Coordination Program Brings Many Professionals Together to Help You
At Futterman, Lanza & Pasculli, LLP, our Care Coordination Program works with clients and their families from the beginning of their journey with us. Our goal is to provide peace of mind for families and their loved ones throughout the process. We assist people in Long Island (Nassau and Suffolk Counties) and metro New York state.
Our elder care advocates are experienced social workers, mental health counselors and registered nurses. We work with your family to develop short and long-term life care plans. It can be challenging to navigate the programs and services available to seniors. Our clients report feeling more informed and better prepared in meetings and assessments.
We make certain that clients can take advantage of all the public health care for which they qualify, including:
We work together and advocate on behalf of your family to ensure the best and most comprehensive quality of care. We work closely with our elder care attorneys and paralegals to provide comprehensive advocacy throughout the Medicaid eligibility process. Our elder care advocates are available to provide support and education to clients and their families.
The Care Coordination and Maintenance Program is also available to families for ongoing advocacy and coaching during the Medicaid recertification and managed long term care (MLTC) appeals process. Our elder care advocates provide guidance and advocacy for each level of care.
